As a former patient, you suffer from hallucinations. You will need to use your sense of observation and memory to detect anomalies.
Objective:
If you are hallucinating, take the staircase marked by a bloody handprint. If you are not hallucinating, take the other staircase. If your judgment is correct, the number of floors to descend will decrease, but any mistake will be unforgiving.
